Is extraction of wisdom teeth absolutely necessary?
Wisdom teeth are the last teeth to erupt in our mouth between the ages of 16-20 in our upper and lower jaws. In case of incompatibility of jaw and tooth sizes, lack of space in the jaw, wisdom teeth may not be able to reach the position they should be in the mouth. In this case, they remain impacted in different positions and degrees. However, if there is no jaw-tooth size mismatch or lack of space, wisdom teeth can erupt in the appropriate direction and position. In this case, there is no need to extract the teeth. After a careful clinical and radiologic examination, it is decided whether your wisdom teeth should be extracted or not. In some cases, impacted wisdom teeth are diagnosed by our examinations. In other words, they may not have given you any complaints.
However, the fact that it does not cause complaints does not mean that it does not need to be extracted. Some impacted teeth that do not cause complaints may cause damage to neighboring teeth and cyst formation around them. In this case, the impacted tooth that causes this is extracted.
Can only wisdom teeth remain impacted?
The wisdom teeth are the most frequently impacted teeth among all our other teeth. However, other teeth can also remain impacted. Canines, or canine teeth, are the most frequently impacted teeth after wisdom teeth. Other teeth in our mouth can also remain impacted and can be maintained in the position and direction they should be with the cooperation of orthodontics and maxillofacial surgery. If the impacted tooth damages the surrounding or neighboring tooth or cannot be maintained, surgical extraction is decided.
Frequently Asked Questions about wisdom teeth and impacted teeth
Both tooth extractions are performed under local anesthesia, which we call local anesthesia, by numbing the extraction site with the help of anesthesia while the patient is conscious. In order to extract impacted teeth located in our gums or sometimes in our jawbone, the tooth is exposed through a surgical procedure by making an incision in the gum. The tooth is divided in order to remove it more easily and to cause less damage to the surrounding bone. The extraction site is then closed with sutures. The difference from normal extraction is that the procedure takes a little longer.
Whether the wisdom teeth are impacted or erupted, the pain sensation is eliminated because the extraction is performed under local anesthesia. Therefore, even if a surgical procedure is performed, you will not feel any pain during the extraction.
It is expected that you will experience some pain after the extraction of wisdom teeth. Since a surgical procedure is performed, this result, which is a natural reaction of our body, can be easily controlled with appropriate medications that will be prescribed to you after extraction. If you take your recommended medications regularly, the healing process is overcome without any problems.
Swelling after wisdom tooth extraction is not experienced by every patient. If the position and the degree of embeddedness of the wisdom tooth requires surgical extraction, it is normal to experience swelling after surgery. In order to avoid swelling, we recommend our patients to apply ice to the operation area and to sleep with 2 pillows on the day of surgery. This swelling reaches its maximum size in the first 2 days and then gradually decreases and disappears in 1 week.
If wisdom teeth are under the gum or under both the gum and the jawbone, they are extracted surgically. In addition, wisdom teeth that have erupted but whose roots are very curved and likely to break should also be extracted with a surgical intervention.
After 20 years of age extraction, pain, swelling, bleeding in the form of leakage and bruising may occur. These are expected events during the healing stages. By using the medications prescribed for you after the extraction, we eliminate pain and the risk of infection. The precaution to be taken for swelling and bruising is to apply cold compresses for the first 24 hours. Bleeding in the form of leakage may occur in the first days, constant spitting or rinsing increases this bleeding. It also delays wound healing. Therefore, in order to stop the bleeding, we recommend that you do not spit, do not rinse your mouth, do not put non-sterile cotton-like things on the extraction site and stay away from hot food and drinks. It will be easier for you to eat softer foods for the first 2 days. Smoking should not be used after the operation as it delays wound healing. Oral care should not be neglected at this time either. After the day of wisdom tooth extraction, you should start brushing your teeth and use mouthwashes specially prescribed for you. If the operation is completed with stitches, the stitches should be removed after 1 week.
